The Patient Administration Department is committed to supporting our patients, clinical staff, and employees. Our mission is to help our staff and beneficiaries with anything related to medical records, following regulations and policies. Medical records belong to the U.S. government, and it is our duty to protect them from being lost, changed, or shared without proper permission.

In- and Out-processing
A copy of your orders is required. Medical records can no longer be hand carried to the next duty station. During out-processing, medical records will be forwarded according to Department of Defense and Army Regulations.
Request for Records Transfer
To request medical records from another military medical facility or civilian provider, please provide the provider's name, address, phone number, and fax number if possible.

Release of Medical Records Form
Requests for Copies of Medical Records
We can provide copies of your medical records request via compact disk, DOD Safe, or encrypted email. You can also download copies of your medical records through the MHS GENESIS Portal.
To process a medical records request, you must sign a release of information form. Anyone over the age of 18 years old MUST sign the request. This includes spouses and dependents unless there is an authorization form on file giving permission for someone else to sign for the patient.
DA Form 5018 ASAP Client’s Consent Statement for Release of Treatment Information will be used for authorization to disclose alcohol or drug abuse patient information from medical records or for authorization to disclose information from records of an alcohol or drug abuse treatment program.
Request for Lab Results
You may request a copy of your labs from the Patient Administration Department. If you need labs faxed to another provider, you must sign a release of information form. Anyone over the age of 18 years old MUST sign the request. This included spouses and dependents unless there is an authorization form on file giving permission for someone else to sign for the patient.
